Mimicking Martin's style to avoid an official insert, here goes.
What is it
A free Eye Test at Boots Opticians
How to get it
Join the Boots Health Club at this address: http://www.bootshealthclub.com/healthclub/signup.html. You either need to have an Advantage Card or sign up for one. Details of how to get your free eye test will be included in your Welcome Pack, which will be received around four weeks after joining.
Anything else good to know
Yes! If it does turn out you need a prescription, there's no need to get your glasses or contact lenses from Boots. Even though the eye test was free, you still have a legal right to be given a legible prescription, meaning that you can order glasses at a fraction of the cost over the Internet, from sites such as https://www.glassesdirect.com
Any other benefits or disadvantages
You have to partake in the Advantage Card scheme to take advantage of the free eye test. This means that you receive 4 points per pound spent in Boots (plus other points - see various threads on the Grabbit Board) worth a penny each to redeem in Boots stores. This also means your moves are tracked by marketing companies, but of course you're free to cut up the card and not use it - after you've received your eye test of course! It might also mean you get lots of unwanted email from Boots, but this should be fairly easy to get rid of with an Unsubscribe option somewhere in the email.
PS - Remember, Imitation is the sincerest form of flattery

Also remember D&A. Their free eye test offer is far less hassle.
Just print the voucher, and take it to a branch. Voila.

You're right, D&A is far less hassle, but I missed out on it when mine needed testing cos I didn't have a printer so had to pay £5 to Specsavers instead
Got one now though
So if you don't have a printer, or want a 2nd opinion on D&A's diagnosis, the Boots offer might be a good deal for you. It's only an on-line form, the only (real) downside is having to wait up to 4 weeks for the Info Pack, although obviously this might arrive sooner.
Anyway this deal now means that you have a choice of providors for your free eye test, which can only be a good thing.
Take care
