You just need to say you want to appeal and why, you can send in any further evidence later if you do not have it to hand yet.
It would be a good idea to get help appealing from a Welfare Rights Officer, these can be found at the CAB, Local Council or disability organisations such as DIAL.
The money will stop from the date of the decision.
Once you have put in your appeal you should be able to claim ESA at the assessment rate whilst you wait for your appeal to be heard.0 -
I wonder why a doctor would have done such a thing?0

If you have not already, ask the dwp for a full copy of your esa medical assessment report, they will send it out free of charge if you ask.
As you scored 0, you already will be prepared for a report that is likely to be akin to a work of fiction, dont take it personally, it happens to many, many people - esa was designed to fail more people than IB, and even on IB many people are having similar problems to what you describe.
The report will help with your appeal, as you can see the reasons given why they scored you 0 points, and its quite likely the report will be easily dismissable as any form of credible evidence by the tribunal (or anyone sensible else that can read properly).
Its not just the doctors fault you know, it had to pass via a decision maker also who made the final decision - and will have to pass via a second decision maker before it goes to appeal (as they have the right to reverse the decision to stop it going to appeal, granting you your benefits back if they deem it right).[greenhighlight]but it matters when the most senior politician in the land is happy to use language and examples that are simply not true.
[/greenhighlight][redtitle]
The impact of this is to stigmatise people on benefits,
and we should be deeply worried about that[/redtitle](house of lords debate, talking about Cameron)0 -
The money will be paid up until the time the b office was informed that you had failed the medical, the quicker you get your appeal in the quicker the payments will be restored0
