Word Templates and hyperlinks - question

I have a problem when opening Templates via a hyperlink. Can anyone help please?

When I open the Template itself, the 'blue band' at the top of the screen says Document 1 which I presume means it has opened a 'copy'. Also, there is a Field for the date which says O XX OOOO, (or similar) and updates to today's date when printed (not changing at a later date)

When I open the same document via a hyperlink (am creating an 'Index') the 'blue band' shows the name of the file. Presumably the actual document has opened. The Field for the date says 3 May 2002, (or similar, which I presume is the date the Template was created, way before my time), this can't be changed.

Is anyone able to explain to me in very simple terms how this can be sorted?
