Reclaiming Charges if account is closed

Hi All,
My first bank account was with Halifax when I was 16 and I got charged the £30 odd more or less every month. I left Halifax and closed the account around 4-5 years ago, so am I still able to go back and claim the charges? I may have the account details somewhere, but if I haven't are the bank required to send me the information?
Thanks for your help!
Phil

The bank are required to send you the details from 27th July 2001 under the FSA Waiver on bank charges. You need to provide as much information as possible for them to do so, bank branch(would be useful), date of birth, address and postcode at the time, etc, etc,
Were you 18 years old when you first incurred charges?0 -

Can't really remember, but I wouldn't have been far off, yes. Do I use the template letters as a start and write to the bank first enclosing the information above?

If you could point me in the right direction that would be great! Having thought about it some more, I was probably between 16 and 18 when the majority of the charges kicked in. Would that affect things?0 -
