    Hi there, as bushyh says really so chase them up if you can.
    How long ago did you request the form?

    Just a thought though - the FSCS seem to do everything in batches in regards of posting, so maybe they are sending so many out at a time, so that they are not over-run perhaps.
    Wonder if they could possibly email it by attachment for you?
    If you have a printer this would be a good idea, maybe worth asking.
