hardship case

hi, i have put in for bank charges reclaiming and the bank has put it on hold, i have checked the hardship checklist and believe i could put a claim in for hardship, but what should i write in the letter,

    You need to write why you are in financial hardship. You need to include an income and expenditure form with notices of any arrears that you have(send copies not the original) and send an updated list of the charges.
