The Cosmo link no longer leads to a form - I guess they've had their 1,000 people already. However... the allaboutyou form (link above) is still there so there might still be a chance on that one.

Edit: the Cosmo form's back again - maybe it was just a glitch.

there wernt no submit button on the cosmo site so just pressed
enter an went thru fine 4 me an dont think its the same as the 1st post
as its didnt say ive already entered so totaly diff items i think
