Tesco Instant Access Acc, need help o.o

Ok, so I was completing my online banking registration having received a letter saying my account was open. I got to the part it asks you to print out a signature verification form to sign and send back. Upon attempting to print this out my pc crashed.
I can now not get back to it or find anywhere to manually download it, it's not stored in my printers cache either. How can I get a copy of this form, I don't want to have to have tesco send me one, delaying things further. Is there somewhere I can manually download this form as I've looked on their website and I can not find it anywhere.
