Hi Carole. You can do a number of things.
1/ click here http://www.insolvency.gov.uk/ You can do the forms online at your own pace.
2/ Find out where your (civil) court is and apply for a date and ask for the forms in a paper version.
You will finds loads of info in the above link and we are here to guide you should you need more help.

also open a new basic bank account either with the co-op or Barclays (it takes a bit of time, dont do like me wait for the last minute and struggling to access your money) ) in preparation for your bankruptcy so that on the day you are BR you can received your wage/benefit tax credit and so forth
if you have an overdraft or existing credit card max them out so you can pay your BR fees and also get some money for day to day living expensesBSC number 1830 -

To find your local county court:
http://www.hmcourts-service.gov.uk/HMCSCourtFinder/
Under Forms & Guidance, type in 6.27 for the Debtors Petition form then 6.28 for the SOA
You then have a copy of the forms, that you can start filling in as you can.
Also a help link where you can see an example of the form to fill in:
http://www.ned-cab.org.uk/debtselfhelp/bankruptcy_628_1.asp
When you come across a bit you dont understand come back on here & post your questions.
It does take a while to do, and we went through a forest with draft versions, but you'll get there in the end.
Good luck :-)DMP Mutual Support Thread Member : 3180 -

Be careful with this last bit - the OR might see this as incurring credit with no reasonable prospect of repayment and consider a BRO/BRU.0 -
you may be right JCS but that was the advice given to me by National debtline before i went BR i am talking here of existing credit facility still open to you and certainly not creating a brand new credit agreement !Be careful with this last bit - the OR might see this as incurring credit with no reasonable prospect of repayment and consider a BRO/BRU.
they are quite a few bit of people on this board who have done just that ... how many people have max out their overdraft to pay the OR fee ?
i may add that just before your BR order is made the OP may have no money until her next wage to live on because probably most of it is gone on debt repayment , charges interest and so forth.. i am just offering a practical way for the Op to have a bit money to live off until her situation become vbetter.BSC number 1830 -
Most OR's do tend to turn a blind eye to use credit cards/overdrafts to pay BR fees. But maxing credit cards and overdrafts prior to BR, may be seen as misconduct, and does lead to further investigation (obviously depends on amounts and what the money is spent on). Whilst I am not saying whether it is right or wrong, it is important that the OP is fully aware before making any decisions.0
