Direct Debit not set up by Halifax - I'm being charged

Hi all
So I took the advice of this website and two months ago applied for, and recieved, a Halifax Cashback Credit Card. Now I'm usually pretty good at doing things right, but in this instance it seems that the Direct Debit was not set up, as my card is now blocked and I'm being charged late fees and interest.
I'm 100% confident that if there was a direct debit setup form on the site as I was applying, I would have filled it in. And I did think there was. However when I called them up about it, they say that you have to call up to set up a direct debit... this makes me doubt myself, and if she was right and there was no sign-up then obviously it was my fault for not paying. However, it seems very unlikely that they wouldn't get you to sign up (preferably for them, for minimum charge) for a direct debit on signup.
Can anyone give me any advice? Anyone else signed up for this card (or any Halifax card) that can tell me if there is a direct debit form online on signup? My aims are to get late fee back, interest charges, cashback, and a correction for late payment on my credit file - if it's their fault that the direct debit was not correctly set up.

Called them to set up the direct debit properly and to pay off the outstanding balance, explained that the direct debit was mistakenly not set up and they tell me that they will refund the late payment fee - I didn't actually ask for it to be refunded so hurrah! No need to go through the process of reclaiming. I'm not going to bother pursuing the interest or credit file corrected.
Here's waiting for it to appear on my statement.0 -
