Natwest Hardship Form

Hello guys!
Well I sent off my letters to LTSB and Natwest asking them to consider my case again as I am in hardship - nothing from LTSB but got a letter asking me to fill out a form for financial difficulties from NW.
Has any one got any advice for me before I fill it out and send it back?

Just be honest about what you enter on the form because they can check a lot of things and there are ratios which they use to check whether your expenditure amounts are within reason0 -
