How do you make an icon

Anyone know how to make an icon. I've just written my first rather basic program for windose and by default I just an the standard exe icon - I;ve worked out how I can give it a different icon - but how do I make by own.
ie are .ico files simply .bmp with another name?
or can I use a graphics program to write them?
