Parental responsibility

Hi all, Had my daughters forms come through for starting school in sept .. only weird thing is they have asked for the parents names and also we have to write whether or not we have parental responsibility.
Obviously I do as the mother, but as my partner (my daughters dad) and I are unmarried and do not co-habit, I was informed that he
doesn`t. Apparantly the father of children born before nov 2003 do not automatically have parental responsibilty unless they are married.
So what do I do about this, anybody know what forms (if any) I have to fill in so her dad has the same responsibility as me ??

Basically its a form you as the mother have to sign in front of a court clerk/magistrate. Here is a link to a website which explains it all in great detail: PRA
Giving your partner PR will make him equal to you in the eyes of the law. Its a good thing I think and its great that you are willing to give PR to him rather than use it as a weapon like a lot of mothers do.0
