Please help- dont think credit agreement is enforceable

Good evening
I've just recently received my SAR request back from Lloyds. Provided by the credit card company was what I had asked for- the Asset Application form and Agreement.
The thing is its only one page with the Asset Application form and Agreement titled at the top. Along with my credit card number scribbled on and SOMEONE ELSES CREDIT CARD NUMBER TOO.
ALong the side there are other scribbles and there is a figure scribbled at the bottom that has what I think my limit. It is one page and does not on that page or the opposite side have any precribed terms and conditions.
I have contacted the DSAR Team who inform me that this is all the credit card department had on me. I am unsure what to do next as they had over 45 days to come up with this.
What would be the next step as this is clearly an application form?
