Barclays DSAR address & how much?

Is this the correct DSAR request address for bank charges please?
I have 4 bank accounts with charges for Barclays accounts. Do I send one cheque for £10 or 4 x £10
Also what is the CORRECT address to send for statements please? The last time i did this 6 months ago Barclays did not even reply back.

it's 1 cheque for £10 payable to barclays bank,(I would write on the cheque at the side "for the purposes of DSAR".
