    Hi
Nearly six years ago I changed back to my maiden name from my married name.
I contacted my solicitor who advised me to do it by means of a 'Change Of Name Deed' as it was a quick, cheap way of doing it.
It was done within a week & is a simple document where it states which name you are changing from to and that you will now be using your new name for everything.
I was given the original document & three certified copies which I sent (& have had returned) to official bodies such as banks.
I had no problems at all obtaining either a passport or a driving licence in my new name or changing bank accounts etc.

I hope this is of some help to you.

Six years ago it cost me £30 or £35 but is probably more expensive now.
I just took along my birth certificate with me to the solicitor as I wanted to go back to my maiden name. As I said it was very quick & easy & once I got passport, driving licence, bank accounts & details at work changed I have never had any trouble opening new accounts or with the tax man.
Once you give your new name to your employers your NI no etc will be automatically linked to your new name.

Would have thought that since you didnt change by deepoll in 1st place all you have to do is prepare a letter stating that as of a particular date you wish to be known as ............. Send this to banks, doctors etc. Contact N I and see how you go about changing the name on card after all you have a birth certificate in your new name. Im no expert though so I could be wrong
Cherish those you have in your life because you never know when they won't be there anymore.
