Help with editting PDF forms

I have an order form for my business that I currently send to customers as a pdf, this was originally generated to save on paper as I am trying to be as environmentally concious as possible, but as the form is a pdf I have realised that it can't be editted on screen so the customer still has to print it our and either fax or send it back to me (sort of misses the point).
The form has the terms and conditions of the direct debit on there and I don't want this to be enabled for editting (as it's just the standard direct debit blurb) but I'd like the for the customer to be able to type in their details and send it by via email.
I have used forms like this for things I have bought so I know it is possible, I am a graphic designer so have access to Quark, Photoshop & Illustrator as well as word etc. does anyone know how it is done?
I know there is software you can buy to edit pdf's but this is from the creating pdf's perspective.

Can you not use an online form like http://www.payplan.com/ see down the right
of this page, can you not do something like that?

If you have Acrobat Pro 6, you can overlay fields onto your existing form which can then be saved and the form e-mailed to your client to fill in. They can then open the file, fill it in and then print or e-mail it back. The only thing they can't do is save to disk.
HOWEVER, if you use Acrobat 7Pro to make the form fillable, if your customer opens it with Acrobat Reader version 7, they can do all the above AND SAVE TO DISK.0 -
