Recording accidents

I am looking for a standard form to use in our office to internally record any accidents that occur with the cars in our fleet.

Can someone rcommend a site where I can download a blank document?

    Why not ask your insurance company for a copy of their claim form? Then you'll have all the info required to submit the claim :)

    The fleet of vehicles are in a foreign country so the standard insurance route doesn't really apply, however thanks for the idea :j

    Probably not far away. You might want to just check any local laws etc that might need to be covered and just add it to the end.

    Is it an English speaking country or is language an issue? If the latter then I reckon your best bet is the "Agreed Statement of Facts" or Constat Amiable.

    This is for use in France. I suspect there are other versions though.

    Agree that the Constat is the best way forward.

    For Parisien and Italian users it's best to keep a few hundred copies in each vehicle.:rotfl:
