call connection, to fee or not to fee

Does anyone know if there are any landline or carrier pre select providers who do not charge a connection fee for each call?
I am aware of some companies offering packages which, for a set monthly price, waive this charge if the type of call falls within the deal's "inclusive minutes".
I would be grateful for any information on companies who simply do not top up the call cost with a connection fee.
