Card care and repayment cover

I wonder if anyone else has had this?
I have today received a statement for my Halifax One credit card claiming that I owe £23.78 for the above.
Now I cancelled my credit card nearly two years ago. A year ago I received a similar statement and Halifax claimed my card was not cancelled. They agreed to cancel it there and then but said I still owed for the cover. I paid it for a quiet life.
A few months ago while my husband was on the phone to them about a query with his card, he handed me the phone so I could double check that my card was cancelled. The lady I spoke to confirmed that my account number was not showing on their system and therefore she confirmed that the card had been cancelled.
So having "cancelled" that card 3 times now, imagine my astonishment to receive another statement today
I have no intention of paying it (they have already had an extortionate sum out of my current account this week for the sake of £1 over my overdraft limit so they can whistle for more). I will be on the phone to them first thing tomorrow.
I just wondered if anyone else had had a similar experience and what happened?

What has caused Halifax to claim money is owed?
If it is a CPA then it is up to the cardholder to cancel these or the card will continue to be charged whether 'closed' or not.0
