CD cover templates / inlay cards?

I've recently created a mix CD for a friends birthday, and I want to burn a few additional copies to hand out to other people.
I'd quite like to design my own CD cover to accompany it, just something VERY basic, whereby I can list the tracks that appear on the CD.
Does anyone know of any websites or software that I could use to do this? If it's free to use (or very cheap), even better!
I must stress that this only needs to be the most basic template, I don't intend to add any pictures or anything, all I need is some lines I can use to type the track names out, and an outline so I can cut the right size to fit the CD case.

Open Office Jewel Case insert: http://templates.services.openoffice.org/en/node/2108 just in case you don't have Office and want a free alternative as you suggested.
