Hello Jonathan,
What I would advise you to do, is visit your local Job Centre Plus.
When a parent has a child coming up to 16, on office from the DWP-Dept.for Work and Pensions acting on behalf of the Secretary of State, will visit, maybe ask a couple of questions, and fill in a form so the parent can act.
I would visit the Job Centre, and politely explain that you are now able to manage your own affairs, and ask them to help you sort this out.
Tell them you sent off a form several months ago, and as yet, nothing has changed.
Just remember when you visit, that they way you relate to the staff, will show that you are able to take control of your benefits.
Regards
