Barclays: Financial Hardship in Branch?

Hello!
A very quick question:
I am reclaiming my bank charges from Barclays and have good reason to believe that I satisfy the test for financial hardship. I'm just about to send off the request for a refund.
I understand that as part of this, Barclays will ask me to fill out a budget form to demonstrate my cashflow.
Now, I work directly opposite a Barclays branch and want to know: am I able to get the form from the branch, thereby saving me several days in the process? Then, I can send the form with the original letter.
If so, what is the form? Is it one of their general 'budget planner' forms?
