Carphone warehouse £18 a month?

Hi, just looking for a bit of advice really. I've recently gone through my account to check up on everything and clear some debt up etc and I came across a monthly direct debit to the carphone warehouse for £18 a month, its been coming out of my account for three years now and I have absolutely no idea what its for!
I've just cancelled it with my bank but they've taken over £500 from me and I'd like to know why, about the time it first came out I did buy a phone from CPW and payed a one off fee for a years phone insurance, I paid a one off fee, I certainly didn't signup to anything that was £18 a month, the phone was on PAYG as well. I've done a bit of googling and it seems that the only monthly insurance CPW offer is the lifeline insurance which costs £12 a month not £18, so I'm pretty much stumped. Anyone ever heard of anything like this before or got any tips before I ring them up tomorrow.

you havent checked your bank statement for 3 years ????????
the whole purpose of banks sending statement out on a montly or quartrly basis is so that customer can check for errors0 -

Granted I didn't check, but I don't really you can think say its my fault, cheers for your reply though...
Hollydays, thats the thing it isn't insurance as I didn't take any out or owt like that, find out tomorrow I reckon.0 -
OP - Do you have TalkTalk?
That comes out as Carphone Warehouse and would be about £18 per month for the anytime calls package.0
