NHS glasses voucher and problems proving entitlement joint JSA

Perhaps im being dim but how do you provide proof of your joint JSA entitlement when all letters etc are in partners name ?

I am overdue for an eye test, have made an appointment for next week and the person in the shop told me to bring in proof of our jsa claim to get a free eye test and a discount on glasses - but having looked at the jsa booklet and the jsa letters we have received, it appears to all be in my partners name, my name dosnt appear on anything except for the tax credit entitlement - any suggestions?

ask jsp for a letter of entitlement or your original award lettern should be proof
