disability discrimination act 1955 advice needed

i have a form to fill in ,that say : do you consider that you are a disabled person i.e. someone with a disability as defined within the disability discrimination act 1955 ? please describe that disability......help what does this mean ? :rolleyes:

its now the disability discrimination act 2005 thats in force
Definition " a disabled person is someone who has a physical or mental impairment which has an effect on there ability to carry out normal day to day activities"
Legislation states the effect must be:
Adverse
Substantial(ie more than trivial or minor)
Long term (ie lasting or likely to last more than 12 months)
Legislation also states examples of impacts on day to day activities
mobility
manual dexterity
physical coordination
continence
ability to lift, carry or move everyday objects
speech, hearing or eyesight
memory or ability to concentrate, learn or understand
perception of risk or physical danger
