Equifax - Information still on file over 6 yrs old and after completion of IVA

Hi everyone, firstly apologies if I tend to ramble on a bit but I need a bit of advice on what to do next.
I entered into an IVA back in April 2003 which has since completed. I've waited several months before sending for my credit files from the various CRAs, the first to arrive so far is from Equifax which isn't the easiest to understand. Most of the information relating to my debts has gone from the file as the default dates/start of IVA is over 6 years old, apart from one, a loan from Nationwide, which was included in the IVA.
The information from Equifax relating to the loan looks like this:
NATIONWIDE TRUST LTD (I) Loan Ms Jane Smith (not really my name, sorry to any real Jane Smiths out there)
Terms 72 @ £196 Monthly
Balances Limit £0
Start £0
Current £+7545
Delinquent £8506
Effictive Dates Start 02/02/1998
Delinquent 06/2009
Birth 20/11/1965
Insight Last Updated 02/06/2009
Monthly Status 6666666666666666666666666666666666666666666666
Then there is a list of balances for the last 12 months showing a decrease from £7690 to £7545, no information on payment amounts etc. Just a Credit Limit of 0 at the end of each line of balance.
I can't understand why this information is still on my file after all these years, I had other debts with Nationwide (a credit card and overdraft) which were part of the IVA and there's no information about them. In fact there is nothing showing about anything to do with the IVA apart from this damn loan.
One other small point which is also wrong, there is also a name of a financial associate, just down as S Smith, which I know nothing about as I've never had any joint loans, applications, insurance policies etc. with anyone, so I need them to remove that too.
I rang Equifax about it and after being on hold for an operator for 45 minutes was eventually told I had to "write in" either to them & they would contact Nationwide or I could write to Nationwide direct. She wouldn't give me anymore information than that.
The thing is I don't really want to get Nationwide involved, I want Equifax to remove the information as it's over 6 years since the date of the IVA/default, which is what I thought should have happened automatically.
Any advice on who I should send the letter to and what I should say would be gratefully appreciated.

Thanks for that, I've already got my amended version of the letter ready. It's not that I'm in a rush to apply for any credit, I just would like to know that everything is in order now my IVA is finally over and it feels like such a weight has been lifted off my shoulders. My name is now off the insolvency register so I just want to take this as an opportunity to start afresh.
By the way any ideas what the "deliquent" entry is on the Equifax CRF or is that just their word for default?

I have the same problem with a loan from a trust deed that i was discharged from in 2005.
Do you know if there is anything can be done to have this removed from my credit file?
Im in Scotland if it makes any difference.

One other question, does anybody know if, when Nationwide update their information and correct the default date to April 2003, will it automatically disappear from my CRFs as it will be over 6 years old? I'm very confused.0

I had a similar problem on ny credit file, Halifax had linked me to an address by a name that wasn't mine, and someone I strongly suspect of attempting to apply for fraudulent loans and cc's in my name.
Does it say who put the info on there? If it does firstly call them, I doubt they will be able to help though if it's been on there a while.
I then contacted Equifax, told them there was incorrect info on my file, who put it there and why it shouldn't be there. They were great they contacted Halifax and told them to remove it, and put a notice of correction on my file while Halifax were investigating it. I did it all through email from the online help thingy.

Thanks Peachyprice, the odd thing about my association is that it was put there by the Next Directory. I do have an account with them myself but it's always just been in my name so I don't know why it's appeared. I've tried to contact Equifax using their on line help thing, but it just won't work on my laptop for some reason so I have to do everything via the traditional snail mail route.

it's not your laptop it must be there system I tried to raise and online dispute with them yesterday as well and would not work , postoffice it is lolTOTAL 44 weeks lose. 6st 9.5lb :T0
