P85 form, tax refund on leaving the UK

After living abroad near 2 years now, I've finally just got off my bum to make this same application a couple of weeks ago.
1) The P85 form was the only one I was told of, and could find in relation to this, and thats after a few calls and letters to HMCS to find out my options. If it were me I would enclose just an extra page/letter in with the P85 form detailing the savings too.
2) Yep, the way I read this was to put your name, ignore the address parts and just complete the bank account info fields also. If you're including a letter you could put a note about this on the same page also - wouldn't hurt, but it shouldn't be needed. I've opted for them posting me a cheque myself but in hindsight should have done the same to get the money a little sooner!
