If you're doing home testing, you just give your date of birth and sign the form. It's only if you're going to the Evaluation Suite in Nottingham that you have to fill in the medical forms.0

I started with Boots ages ago, and I needed to fill ALL the forms out..... they said in case I had any allergies etc so, has it changed now??

I've been doing it since August last year, and when the medical form arrived in the post it said you don't fill it in if you're doing home testing. When you finally register online, you have to give a lot of information about the cosmetics and skincare you currently use, and name everything. You also have to state if you have sensitive skin and are allergic to anything.0

dmbw I dont think they are looking for anyone. I dont know if they still send the forms out anyway for some kind of backup but as far as I know they havent been accepting new peeps since last summer when I joined.What Would Bill Buchanan Do?0

Just received lip gloss and nail polish. lip gloss in a pretty petal container.....nail polish nice......but I can't open the bottle!!!

Phew!!... Finally got it open [with aid of pliars!] Not sure it was worth all the effort tho! Takes AGES to dry, and the colours a bit naff lol....not very shiny either. Can you tell, I'm not too impressed? lol
