you should contact them... you should get the forms with each product..... if you get something that needs numerous forms then you should get them all at the same time (with the product!),

It has been known to get the product and no form...or more likely, the form and no product!

Check the forms as the ones I got asked if I'd keep using the product for "another" two weeks so referred to the product I'd already received.

I've just received handcream trial UT7399. I recognise the fragrance but can't put my finger on what it is. Anyone else get it?

I have this- I just took a whiff and the first thing that came to mind was 13. (The age). Then it clicked- it smells just like the Impulse body spray Temptation- used to wear this all the time when I was that age!
