Yorkshire Bank Credit Card Reclaim - HELP!

Can anyone help me, I feel like I'm trying to find missing parts of a puzzle! My mum had a Yorkshire Bank credit card that she was paying off slowly - too slowly it seems for the bank as they sold her debt to Cabot without a word of warning...but that's another story. I said that she should try to claim any PPI and/or charges to her credit card and that I would do it for her. She thinks the card number was 4509xxxxxxxxxxxx - any YB customers on here able to confirm that their card numbers start with those numbers? And secondly, anyone got an address for Yorkshire Bank Credit Card HQ? My mum hasn't kept a single statement so she isn't that much use but is in dire straits financially so I want to help her if I can.
