I have tried searching for this could find other samples for it but not from this website. Apologies if it's a duplicate.
I've just received an email from www.myweekendmyway.com about competitions they are doing, but also if you do a short survey (1 min ish)They are giving you trial packs of tampax pearl.
Free trial offer open to UK residents only over 18 years of age. P&G will not store your address for future campaigns. To view the Procter & Gamble privacy policy, please visit: http://www.pg.com/privacy.html
P&G will not store, share with 3rd parties or use your address details other than to send you a free pack of new Tampax Pearl if you qualify for the trial.
I got a message on the next screen saying i had qualified, but you do have to put in a telephone number and agree to mail from myweekendmyway.

Congratulations message here! I have qualified!
You just have to be a bit judicious in your answers - ie make sure you have lots of friends, go out a lot, blog a lot, phone and text a lot etc etc. Oh, and letting them know you are 18 - 24 helps
