credit card ppi help

Sorry but I don,t know how to do this. Can anyone help me with regards to claiming ppi from Lloyds TSB. I applied for charges back last year and got a final letter saying no. Is it too late to go to the FOS or do i have to go through the process of asking the bank again or have i left it too late. Can anyone give me some advice please. Thanks!
