PPI claim letters, what to include

I have recieved a rejection letter from my Capital One credit card insurance due to a pre existing medical condition. I took the policy out in 2002 was'nt asked any questions about my health at that time. My medical report shows I had had symptoms in 2000 related to my current illness so they have rejected the claim. I have been ill over 12 months and should have had the full payout. This is my first ever claim on the insurance.
I am using the step 1 letter template to claim for the policy being incorrectly sold to me, but is it advisable to include the fact that I have been rejected by the insurance co, or should I not discuss that in the letter? Has anyone any previous experience that can advise me?
