I am from Spain now work in uk for a business project , I had problem with my English writing ,any people can give me a genuine help not a laugh, thanks again.Please corect my syntax for the following short letter . i will send this letter to my clinic specialist regarding my previous bladder disorder etc symptom, i dont know how to make this letter look formal and polite ,please re -write in a nice way and same contents or just correct the syntax.

Thanks for your care and professional consultant by the first appointment few months ago ,now I received the letter which regarding the next appointment for Flow rate and Bladder scan on Monday 8th June . I felt little new symptom recent days , I have to go to the lavatory again when i just had urination 20 minutes ago frequently ,my bladder cant hold lot water as before, it only past small amount of water when I felt full bladder. And I felt the water going past away from my urethra in interval second since week ago because the mild irritation . I felt numbness of the right foot during the day and night for couple hours each day starting on 6th May. So can I ask the nurse scan the whole Urinary Tract ( i scanned kidney ,bladder last time,but no prostate,urethra,down part) when they scan my bladder in that day ?

Dr Jane,
First of all let me thank you for the professionalism I experienced during my last appointment with you.
I have recently received a letter requesting I re attend for a further appointment in regard to a ‘flow rate and bladder scan’ scheduled for Monday the 8th June.
However, recently I have started to experience more symptoms which I believe may be of concern. For instance, it seems I can only hold water for 20 minutes before I need to urinate which is unusual and somewhat worrying me. Particularly since when I do actually urinate I only pass a small amount and it is quite uncomfortable in that I can feel the urine passing my urethra.
I am unsure whether or not this is related but I have on several occasions had numbness in my right foot, this started on the 6th May and has been both during the day and at night.
Last time I had my Kidneys and bladder scanned, however it is my request that with your approval this time I can have my entire Urinary Tract scanned including my prostate.
I hope you can give this careful consideration and I look forward to seeing you soon whereby I will have a better opportunity to further explain my symptoms.
