International/Offshore Bank Account

I'm looking for a safe international/offshore bank that:
- allows me to hold my money in U.S dollars
- allows me to easily convert between other major currencies
- is cheap to do various transfers and to receive money
- allows me to bank online
- allows me to withdraw money via an ATM anywhere in the world.
- no or low monthly charges
- a low minimum\opening balance (£5000 or under)
So far HSBC International looks like the best bet. Any others?

Most UK banks will offer you what you want but of course they will have their own charging regime.0
