Bank Letter, please help

Hi, i am in financial hardship so want to reclaim my charges, i haven't got my statements and when i went into the bank they said to write them a letter that they could send to the main Nat West bank to request a list of my charges, rather than paying for the statements, so what i need to find if anyone has any idea, of a letter that i can send to the bank or if i could just write please could you send me a list of all the charges that i have incirred over the last 6 years. any help is gratefully recieved,
