After free address book/to-do list/organsier software

Hello. I'm after a bit of free software which allows me to do the following:
Keep a database of names/addresses and phone numbers - PLUS a section to write notes from meetings and phone calls etc.
Set reminders, so if a customer asks me to call them in two months, I can set it on the software and it'll pop a reminder up when the time comes.
Store info online, or allow me to take the data home on a memory stick so I can transfer between home & office.
I've started to input stuff into Business Contact Manager in Outlook 2007 that I have on my office PC, but when I work from home I use a laptop with an old outlook on, and my company won't upgrade to 2007.
I was hoping there would be an open source package out there. I've had a search but not come up with anything that seems appropriate.
