As someone who supports people with the completion of application forms, the actual information is always best but a compromise such as, "Assault related conviction with details to follow (awaiting police record detail). I would welcome the opportunity to discuss this unfortunate circumstance in full at interview."
It doesn't sound great mentioning assault but whichever way you look at it you can't escape the facts. Encourage him to tackle it head on, if he wants to work in the NHS he is going to have to lay his cards on the table.
*EDIT* - As well as the enhanced CRB he may well have to go through a POVA (Protection of Vulnerable Adults) check. I don't know if there is a similar one for children.
Thank you for replying, he is just going to be truthful. It was a silly thing he did, he knows this and hopefully they will see it as he was open and honest and at least give him a chance.

Could I ask how long ago the incident was? It will be taken very seriously by the employer, but if it is a long time ago it is less likely to be a bar to employment.Gone ... or have I?0

It was only about 3 years ago. The whole thing was a farce though, even the judge asked why he had pleaded guilty (he went against his solicitors advice). He has sent the form off now, will just have to see what happens. From my experience working within NHS HR, the view that is taken is that if you are as honest as possible from the outset, from the application form to the interview to the induction etc. when the CRB disclosure is received your honesty will be treat favourably and most minor offences don't jeopardise employment.
As far as what to write on the application form, give the year of the conviction and perhaps state 'assault - pending clarification from police' then explain further at interview. If you put "I have a conviction but am unsure of the exact naming, i have requested this information from the local police force and would be willing to provide this when i recieve it" it's very vague open to all sorts of interpretation and it would be unlikely your application would be shortlisted.
The NHS are exempt from the Rehabilitation of Offenders Act 1974 and Porters are put through an Enhanced CRB check as they are involved with vulnerable adults and children (so everything is recorded now, even if there was no conviction, if there's been any conflict where the police have been involved, it's likely to be noted as 'additional information' - which has come as a result of the Ian Huntley case).
Honesty is the best policy to avoid being disciplined for 'dishonesty' when your CRB disclosure is received.

I would suggest that such things are best put in a sealed envelope attached to the form - you say "yes" to the "Do you have any criminal convictions?" question but then put the information in an envelope rather than on the form, in plain sight.
That way your form should be assessed on its merits and the disclosure only considered if it's actually relevant.
Whilst I can certainly see the reasons behind doing this, applications are often opened and sorted by admin staff and passed on for sifting. Any chance of attached documentation becoming detached is a risk that, in my opinion, outweighs the reasons behind the need to keep things overly discreet.
It however nice to have alternative options and if someone is very concerned about who might see information then of course if could be considered. Don't forget there are companies out there who do not like attached information such as CVs, etc and want all information on the form that they send out.0
