Application form and criminal record history.

My partner is filling out an application form and needs to declare any criminal convictions. He has been arrested and charged etc but cannot remember the exact conviction. He did go to court and had a fine and a years conditional discharge i think it was. I have contacted the police and it can take 20 days to get the information as he has to fill in a form and send it off. The job closes next week and he doesn't want to leave it blank as he will need a Crb check.
What is the best thing to put on the form
I was thinking put something like, " I have a conviction but am unsure of the exact naming, i have requested this information from the local police force and would be willing to provide this when i recieve it".
Any other ideas ?

He must remember what he got, or at least what the outcome was and the date it was handed down for god sake?. That would be sufficient as at least. If not, put awaiting information from police.0

He knows what the outcome was but cannot remember if it was ABH, GBH or assault. OK, if you know the outcome put that down and a date for example "June 2007" doesn't have to be day specific. Put down as much as poss that way you'll be ok!0

I have no idea why he doesn't know the exact charge, nothing was told exact though and lots of charges etc were discussed at the time.

And don't forget, unless it's a reserved job (working with kids etc) he doesn't have to declare it if it's 'spent' under the Rehabilitation of Offenders Act. I can't remember what the rules on that are, but you should be able to google it.:idea:0

Ah, yes, then he probably does, but check the job description anyway. Unless it says so, he doesn't have to.:idea:0 -
All the jobs with the NHS entail having a thorough CRB check nowadays - so all convictions have to be declared0

I think you will find he will need an enhanced CRB check if he is applying for work as a porter as he will come into contact with kids and vulnerable old people.0

As someone who supports people with the completion of application forms, the actual information is always best but a compromise such as, "Assault related conviction with details to follow (awaiting police record detail). I would welcome the opportunity to discuss this unfortunate circumstance in full at interview."
It doesn't sound great mentioning assault but whichever way you look at it you can't escape the facts. Encourage him to tackle it head on, if he wants to work in the NHS he is going to have to lay his cards on the table.
*EDIT* - As well as the enhanced CRB he may well have to go through a POVA (Protection of Vulnerable Adults) check. I don't know if there is a similar one for children.0
