can you get dla for depression and ocd?

Can I claim disability premium ? I phoned up for the forms but they sent me the Severe Disability Premium ones.

How bad is my depression ? in 2004 I tried to kill myself I woke up in hospital with a drip in my arm for 2 and half day's my doctor has put on the dla form 'Mild to Moderate Depression' also in 1993 tried to top myself ended up in hospital that time to (most prob so I wouldn't get it which I never) but my friend said you carn't get lower that! My doctor also put my wear and tear of the spine down to my weight I was a size 14 before I went on anti-depressants had to stop taking them because I put on half a stone a week on .. should I change my doctor's ? my depression is that bad now I dont know who I am anymore, I don't do anything like I use to, just sleep all day, don't bath anymore (use to bath every night) social service's are going to put my 2 boy's on the at risk list coz I carn't cope, haven't been out of my house since oct 2006 only to the doctors.
