0Spam - block list

I've been using 0Spam (https://www.0spam.com/) for some time now and find it pretty effective, and it's Free!!

Anyway I was reading what the site had to say about configuring a block list and it says:
We do not recommend the use of the E-mail blocklist to block spammers, as most spammers use E-mail addresses that change each time they send out mail!
Now I never though of this before but how do you actually go about blocking these professional spammers from addresses abroad who may be using a dot com address, easy enough if it is dot ru or something like that.

How do members construct thier block list, what spam software do you use, is it Free?
