Is your company large and formal, or small and informal?
But generally - you want to include your expected date of leaving for maternity and when you hope to return to work, alongside a request for reasonable adjustments to be made. I'd also request a meeting with an HR person if you have one, otherwise a meeting with the relevant H&S officer to discuss your 'condition'.
If you've got any special requests, it's also worth including them in the letter. For instance, if you find that you're suffering from morning sickness - then you should ask if it's possible to move onto 'flexitime' while you're pregnant.

No, no, no. Don't do this - it leaves you no paper trail, and if the boss in question is resentful towards your pregnancy (and many are!) - then he can be very awkward in private and you'll have no paper trail to back up any potential legal action in the future.
A letter avoids all doubts and allows the employer to clearly see what might be needed. From Poland...with love.

Also you don't actually have to tell them until 25 weeks unless you want to, or your jobs physical i didn't tell my employer until about 15 weeks. I didn't want the baby fuss and wanted to get the 1st trimester out of the way just incase
My first letter just basically said i'm writing to inform you in pregnant with an expected due date of *inserts due date* and that was about it. They were really good with risk assesments and little chats throughout the pregnancy and any other requests or anything were put forward in these meetings. Followed by a tonne of paperwork, i'm sure theres too much paperwork in some places lol0 -
