child benefit forms

does anyone know of a email address or anything i can use to request child benefit forms? OH went on a "tidying" spree on returning from the hospital and somehow managed to lose it from our bounty pack, i went on the website and its got a print off thingy but we dont have a printer, and having just had a phone bill of double what i normally pay from being on hold to benefit offices and banks etc i dont fancy having to call them unless i really have to!!
