Overseas dividend tax reclaims for UK residents

I'm having difficulty finding the forms issued by the Swiss and Spanish tax authorities which a UK resident can use to reclaim a portion of the withholding tax placed on my dividends. 
This is for companies based in Switzerland and Spain, where the dividend withholding tax is higher than the UK, so we're entitled to claim some of the difference.:j
Any assistance would be much appreciated.
