Dla getting doctor to fill in form ?

It has been 9 weeks since the DLA wrote to my GP about filling in the report, yet they are still waiting for a reply.

I left message after message with my GP to please fill out the form and send it back, yet still I wait.
Even the DLA said that it is very unusual for the specialist report to come back before the GP's report. She is my regular doctor and she knows everything about me. How can I get her to finally fill in the form and send it off before she jeapodises my claim?:mad:
