I filled out the form for myself, my mom & sister earlier, then I noticed how many replies this post was receiving, now I'm really worried about identity theft, where did this offer come from? Was it from the magazine? Perhaps Grazia should be contacted if it is legit to advise their offer is coming up the an "identity theft warning"???
