Information needed on finding a lost premium bond...

My dad has one solitary premeium bond from when he was about 16, is there anyway we can find the number for it? As we've lost the actual paper version they send you...
Also, anyone know what the chances are that the bond has money waiting to be collect as it's been his for a good 40 years now.

Edit: Ok so we've found the paper permium savings bond and we have a premium bond number, but we need his holders number to find out if there are any unclaimed prizes on his bond, anyone know how we can get it?
