Help filling in a form for A&L

I have been reclaiming charges on behalf of my partner from A&L and although we sent them an income and expenditure they have sent a form for him to fill in for ins and outs.
My question is, as his account was in his sole name do I fill out the form for the income and expenditure on his own or for our household?
The reason I ask is because they want to see payslips and p60 but he has been out of work for over a year now and has neither, in other words he has zero income. He does have a couple of debts but I have been paying those.
They also want to know about child benefit and tax credits but beacuse we only get tax credits because I work do I include it on his form?
Sorry if I sound a bit stupid but I just want to make sure I fill it in correct before I send it back.

Is the account in joint names? Are there mortgage/rent arrears? Are there council tax arrears? Are there utilities arrears?
I suspect they will need it for the household as that is how they may decide on hardship or not. Might be worth calling them and asking them just to be certain.0 -
