Providing proof of financial hardship

My husband have previously put in a claim for bank charges. My bank have now asked me to provide them with information proving I am in financial hardship and have given me a questionnaire to fill in.
What classes me as in financial hardship? I have several debts with debt collection agencies and have payment plans in place. I also have been threatened with bankruptcy although now a payment plan is in place.
